Frequently Used Forms
PERMISSION SLIPS
A Parent Permission Form is REQUIRED for every activity or trip, for EACH girl, whether parents attend or not.

Health History Form
This form is required annually for each scout.
Annual Parent Permission Form
This form obtains parent/caregiver permission for all troop meetings, events, and activities for the Girl Scout year, excluding extended-day trips (8+ hours), overnights, and/or high-risk activity as defined by Safety Activity Checkpoints.
Product Programs Parent Permission Responsibility Agreement
This form is required for each scout annually to participate in Fall Product / Cookie season.
Annual Permission Notification Form
This form is used to notify parents/SUM of activity outside of the normal meeting time and location but not requiring a single use permission slip.
Parent /Guardian Single Use Permission Slip
This form is required for every Extended-Day/Overnight/High-Risk activity or trip.
